ACL rehabilitation has evolved well beyond a simple post-operative timeline. The athlete ultimately has to tolerate the speed, force, repetition, fatigue, decision-making and unpredictability of their sport — and a rehabilitation plan must be built around that reality.

At Physio Tullamore we follow a structured progression: restore basic knee function, rebuild physical capacity, and then express those qualities in genuine sporting situations. Our approach combines the performance-rehab structure of Murdoch, Harries and Bakker with Matthew Buckthorpe's evidence-informed work on knee-extensor recovery, task-based progression, plyometric reconditioning and return-to-sport preparation. Biology is respected, progression is earned, and gym strength is deliberately transferred to running, jumping, cutting and competition.

The Eight-Stage Criteria-Based Pathway

Each stage is passed on measured criteria, not on the calendar. The next demand is only introduced when the athlete has earned the capacity for it.

Pain and effusion control, restoration of extension and flexion, and a normal gait come first. Extension must be active AND passive — usable during stance and walking, not just demonstrated on the treatment table. A swollen or stiff knee suppresses quadriceps activation and encourages protective movement strategies.

We retrain weight acceptance, stance time, knee extension in stance and swing-phase knee flexion. Buckthorpe places knee-extensor recovery at the centre of ACL rehabilitation because quadriceps size and strength deficits persist into return to sport and are associated with poorer mechanics and knee function. Early strategies include appropriately dosed isometrics, progressive resistance, neuromuscular electrical stimulation and, where clinically appropriate, Blood Flow Restriction (BFR) training.

Following Murdoch, Harries and Bakker, the priority is first "stopping the drop", then restoring and developing output. Sufficient muscle size and maximal force capacity must be in place before higher-speed qualities can be expressed. Progression is guided by joint response and graft-site considerations — not by the rehabilitation calendar.

Sport asks athletes to produce and absorb force within very short time windows. Controlled isometrics progress to faster force application and rapid maximal efforts — bridging slow gym strength to acceleration, landing, braking and cutting.

We follow Buckthorpe and Della Villa's four-stage criterion-based plyometric programme, manipulating demand through effort, momentum before ground contact, contact time, stance and training environment. The progression runs from controlled bilateral landing and propulsion to unilateral, multidirectional, reactive and higher-speed tasks. Jumping prepares the knee for the different roles it performs in sport.

Running is not the finish line. Acceleration, deceleration, change of direction and high-speed running each place different demands on the knee and should be trained deliberately — not left until team practice.

Predictable drills progress to faster, more demanding work while maintaining quality, and then to variability, reactive decisions, opponents, contact, fatigue and sport-specific skill. An athlete can look excellent in a closed drill yet struggle when they must perceive, decide and act under pressure.

"Fast, Fit and Skilled": perform high-intensity actions with quality, repeat them under fatigue, and execute sport-specific skills and decisions under pressure. Return to team training, return to competition and return to previous performance are not the same milestone.

Do not progress because the calendar says so. Progress because the athlete has earned the capacity for the next demand.

The Performance Triad

Return to performance is built on three qualities that are developed TOGETHER, not sequentially:

Strength & Power

Muscle size, maximal strength, eccentric capacity and rate of force development — the engine behind every sporting action.

Movement & Reactive Capacity

Landing, plyometric and reactive strength, plus cutting, braking and change of direction under real sporting conditions.

Fitness & Skill

Conditioning to repeat high-intensity actions, and the sport-specific skill, decision-making and confidence to use them under pressure.

What Should Return to Performance Look Like?

  • A quiet knee with full, usable range of motion and normal gait.
  • Restored quadriceps and lower-limb strength with appropriate graft-site capacity.
  • Sufficient eccentric strength, explosive strength and rate of force development.
  • High-quality landing, plyometric and reactive strength across bilateral and unilateral tasks.
  • Progressive exposure to acceleration, deceleration, change of direction and high-speed running.
  • Conditioning to repeat high-intensity actions through training and competition.
  • Technical skill, decision-making and confidence under pressure, fatigue and unpredictability.
  • A graduated return through on-field rehabilitation, team training, competition and ultimately previous or improved performance.
