The Problem With Timelines
Two Athletes. Same Week Post-Injury. Completely Different Athletes.
Returning to running following a hamstring injury should not be based solely on time since injury. Two athletes can both be eight weeks post-injury and be nowhere near the same level of readiness. One may have full strength, no pain and clean running mechanics; another may still have pain, a significant strength deficit and poor tolerance of loading. Treating them identically because of the same post-injury date does not make sense.
Research by Fearghal Kerin and colleagues, demonstrates that hamstring injuries can have very different rehabilitation
timelines depending on their anatomical characteristics. In professional rugby players, injuries involving the intramuscular
tendon
were associated with a substantially longer time to return to full training than injuries without tendon involvement.
This is why rehabilitation should be individualised rather than dictated by a fixed timeline — and why the decision
to run should be criteria-led, not calendar-led.

Running is rehab
Running itself should be used as a rehabilitation tool, introduced early in controlled doses — not withheld until the end of rehab and then "switched on".
Progress by objectives

Speed is the stressor

Sprint speeds, repeated-sprint volumes, acceleration/deceleration loads and movement qualities of a senior club, academy or county athlete are not the same. Your targets come from that profile, not a generic protocol.

Build the capacity
We rebuild your physical capacity and conditioning back to that level — eccentric hamstring strength, high-speed force production, tissue tolerance and repeated-effort conditioning — measured objectively at every stage, not assumed.

Coach the movement
Working on the technicalities of how you move: stride mechanics, posture, ground-contact strategy, acceleration and deceleration technique. Passing a strength test while running with compensations is not ready — how you run matters as much as whether you can.
Once the criteria are met, running is reintroduced through a structured speed and exposure ladder — progressing only when the previous stage is tolerated:

What We Look For Before the First Run
Kerin's research into return-to-high-speed-running decision-making supports a multidimensional, criteria-based approach. Before running is
introduced, the athlete should demonstrate:
1. Pain-free function
Minimal or no pain during everyday activities and rehab exercises. The absence of pain is considered an important criterion by the majority
of elite practitioners — it should not be ignored because a particular date has arrived.
2. Eccentric hamstring strength
Strength progressively restored, with particular attention to eccentric hamstring strength — a key criterion when progressing towards high-speed running, built from controlled strength work towards demanding eccentric loading.
3. Progressive exposure to running
Running introduced gradually — lower speeds first, then faster running, acceleration and sprinting — progressively exposing the athlete to both internal and external training loads.
4. High-speed & maximal running
A major milestone: reaching high or near-maximal running speeds without pain or adverse symptoms — essential in Gaelic games, where athletes repeatedly accelerate, decelerate, change direction and sprint.

Real-World Example
The Luke Loughlin Case
In April 2026, Westmeath footballer Luke Loughlin suffered a Grade 4C hamstring tear and tendon rupture during Westmeath's Leinster Championship quarter-final victory over Meath. He required surgery and initially believed he would probably not play football again during 2026.
Instead, in August — just over three months after injury and surgery — he came off the bench for his club, The Downs, and immediately contributed two two-point scores and helped create a goal in a 1-13 draw with Coralstown/Kinnegad.
His return demonstrates why time alone cannot be used as the primary return-to-running or return-to-play criterion. A three-month return following a complete rupture should not be interpreted as an appropriate timeline for every athlete — it shows what is possible when rehabilitation, physical preparation and individual response to loading allow it. His manager also highlighted the significant prehabilitation and rehabilitation work completed around the surgery.
Loughlin's experience should be viewed as an individual case study rather than a target timeline. Significant hamstring injuries have highly individual recovery timelines; successful return depends on the capacity to tolerate progressively increasing loads — not the number of weeks since surgery.
